*Adult leaders DO NOT stock any medication.  If prescription and/or non-prescription medications (i.e. Tylenol, Halls, Advil, etc.) are needed or might be needed, please send them with your child.  Place these medications (in their original container) and written physician’s instructions on paper in a plastic storage bag with your child’s name on it. 

We, the Christian community of St. Malachy parish church and school, are determined to protect all children, from birth through age 18, when they are entrusted to our care for catechesis, extracurricular activities, or recreational sports. We attempt this by adhering to Safe and Sacred practices whereby we shall partner with parents in a uniform handoff practice where there is an established and communicated exchange between the parent and the activity facilitator for the various activities.

I give permission for my child to take part in the St. Malachy sponsored activities and programs for 2016-2017.

      In consideration of the opportunity for my child to participate and fully recognizing that such an undertaking involves an element of risk, we assume all risks and hazards incidental in such participation and do hereby release, absolve, indemnify, and agree to hold harmless the Archdiocese of Indianapolis/St. Malachy Parish its agents, employees and officers, the chaperones, leaders, organizers, sponsors, and persons transporting our child to and/or from these activities.  Neither the Archdiocese of Indianapolis/St. Malachy Parish nor any of said persons shall be held financially responsible for any injury, illness, or death incurred as a direct or indirect result of the activity.

      We, the undersigned, have read this release and understand all its terms and execute it voluntarily and with full knowledge of the significance.

      In the event of an emergency and I cannot be contacted, I hereby authorize emergency treatment to be administered.

St. Malachy’s Shamrock Summertime Payments Made Easy

St. Malachy requires automatic payments.  The process is very simple and will have you writing one less check each month, it will also eliminate the possibly of late payments for you.  Simply fill out the Automatic Debit Authorization Form below and turn it in with your registration.  Each week, on the Thursday before a registered week, your payment will automatically transfer from your banking account into St. Malachy’s bank account.  If you do not have sufficient funds in your account to cover the withdrawal, you will receive a non-sufficient funds notification and be required to pay the fee in person along with a $20 surcharge.  The first transfer will occur on May 26, 2017 for week #1 and the last transfer will occur on July 28, 2017 for week#10.  Fee payments are not subject to adjustments due to illness, vacation, absences, or weather closings.  If you have any questions or concerns about Automatic Shamrock Summertime Payments, please contact John Kiefer in our Business Office, 317-852-3195, ext. 7101.
